Do you offer telehealth or in-person sessions?
I offer in-person sessions and telehealth sessions for anyone located in the state of Illinois, and telehealth sessions only for those located in the state of Texas. My office is located in Chicago, close to public transportation and with free street parking nearby (1700 W. Irving Park Rd., Chicago, IL).
Do you take insurance?
I am in-network with Blue Cross Blue Shield PPO, Blue Choice and United Health Care. For those with other insurance plans, I am happy to provide a detailed bill for services for you to submit for out-of-network reimbursement.
I always recommend contacting your insurance carrier directly for the most accurate information on your fees for therapy. You can do this by calling the member number on the back of your insurance card and asking about your behavioral health benefits, and to confirm that I am in-network with your specific plan.
What questions do you recommend I ask my insurance carrier prior to my first therapy session?
Is Jordan Chaplik an in-network provider for my plan?
Do I have a deductible to meet before my copay or coinsurance kicks in? If so, how much is remaining on my deductible?
What is the copay/coinsurance per session after I meet my deductible?
Are there any session limits?

Do you have sliding scale options?
Yes, sliding scale and payment plan options are also available based on client need and therapist availability. Please feel free to reach out to learn more about these options.
What are your credentials?
I am a Licensed Clinical Social Worker in the state of Illinois and the state of Texas. I received my Masters in Social Work from the University of Chicago in 2018, and completed an Acceptance and Commitment Therapy Fellowship with the University of Chicago in 2024. I received my undergraduate degree from the University of Texas (go longhorns!).
Are you involved in any other organizations?
In addition to working as a therapist, I am also a Clinical Director of the Michigan Boys’ Program at Experience Camps! Experience Camps is a free, national week-long grief camp for kids 8-16 years old who have experienced the death of a parent, caregiver or sibling.
